The file formats are explained in the OptiFDTD User Reference Manual Appendix B: File Formats. Attached is a snippet of a file used for an Input Plane while running a 3D simulation. I have also attached a sample file that could be used as the starting field. Make sure that the number of mesh points in the simulation and in the file are the same.
