If the constellation diagram looks fine and the eye looks very clean as shown in the image you’ve attached, then there is an issue with setting up (connection) the BER test set.
It is too difficult to provide guidance without have access to your project. Will you be able to email it to me at ahmad.atieh@optiwave.com as a zip file, and I’ll try to help?
