Setting the CW laser with a azimuth of 45 degrees will create an optical signal with equal parts x and y polarized. I just noticed your polarization controller after the CW laser which is setting the polarization back to x. Check all your other components to make sure they are manipulating the polarization correctly. The optical time domain visualizer can split the signal into the separate polarizations so you can verify.
