According to your second question, usually the eye diagram describes a statistical distribution of informational bits (“1s” and “0s”) with the presence of noise. Therefore, you have your diagram for one sequence of bits (theoretically this sequence can be measured based on time interval of your observation). It always shows “1s” and “0s” and related noise. If you are not sure about the obtained results of your threshold, you can compare them with the theory.
