Can anybody help me to find the error in design. Some constellation points are missing in this system. The file is attached. also please help me to know the port configurations in the new OFDM block.

The reason for this problem is that the sequence length is too short with regard to the bits per symbol of the system. Each constellation point correspond to a specific bit pattern in your bit sequence. When bit sequence is short, not all bit patterns find the chance to be made and therefore, the above problem occurs. You just need to lengthen the sequence length in the layout parameters (I changed it to 4096).
